Abstract
The aim of this study was to clinically evaluate the effectiveness, stability of color, absolute risk and intensity of tooth sensitivity (TS) from tooth whitening combined with prior application of desensitizing agent. It was a randomized, parallel study, in which 80 patients were selected according to the inclusion/exclusion criteria, which were randomized into two groups: combined bleaching (CA) and at-home bleaching (CC). In the CA group, the desensitizing gel (Dessensibilize KF 2%, FGM, Joinville, SC, Brazil) was applied for 10 min prior to in-office bleaching. After an in-office bleaching session with 35% H2O2 (Whiteness HP Maxx, FGM), two 15-min applications were performed. The next day, at-home bleaching with 4% H2O2 (White Class with Calcium 4%, FGM) was started for 30 min twice daily for three weeks. The CC group received only at-home bleaching with 4% H2O2 (White Class with Calcium 4%, FGM), 30 min twice daily for three weeks. The color was initially recorded during the active phase of bleaching (after the first, second and third weeks) and post-bleaching (one week, one month and six months post-bleaching) with the Vita Classical (Vita Zahnfabrik) scales and Vita Bleachedguide 3D-Master (Vita Zahnfabrik), and spectrophotometer Vita Easyshade (Vita Zahnfabrik). The TS was recorded through visual analogue scale (VAS 0-10) and numerical scale (NRS 0-4). The TS in CA group was evaluated immediately, up to one hour and 24 h after dental bleaching in-office and daily during the 21 days of at-home bleaching. In the CC group, TS was evaluated daily during the 21 days of bleaching. The data obtained were analyzed using appropriate statistical tests (α = 0.05). There was no statistically significant difference between the groups for the absolute risk of TS (p = 0.19). The TS intensity was higher in the first week for the CA group (p <0.05), with reduction throughout the treatment. Dental bleaching was effective for both groups, and there was no significant statistical difference between them when the color was evaluated using the Vita Classical scale (p = 0.118) and the Vita Easyshade spectrophotometer (p = 0.654). There was a statistically significant difference between the groups when the color was evaluated through the Vita Bleachedguide scale (p = 0.043). The degree of bleaching obtained using the instruments used remained stable after six months of treatment for both groups (p> 0.05). It can be concluded that both techniques were effective and stable after six months of treatment, but a greater degree of bleaching was observed for the technique associated with the Vita Bleachedguide scale. There was no difference in the absolute risk of TS among the techniques, but the associated technique resulted in a higher TS intensity.
