Abstract

The aim of this study was to evaluate the effectiveness, absolute risk and intensity of tooth sensitivity (TS) resulting from in-office dental bleaching after prior coadministration of Ibuprofen 400 mg and Caffeine 100 mg. It was a randomized, parallel, triple-blind, placebo-controlled clinical trial in which 84 patients were selected according to the inclusion and exclusion criteria, and were randomized into 2 groups (n = 42), according to the medication administered: Ibuprofen and Caffeine group (ICG) and Placebo group (PG). The groups received 1 dose of the medication 1 hour before the dental bleaching and 5 more doses after, each of the doses administered every 8 hours, for 48 hours. Two dental bleaching sessions were performed with hydrogen peroxide (HP) 35% gel (Whiteness HP AutoMixx, FGM), with a single application of 50 minutes and a 1-week interval between sessions. TS was recorded during the dental bleaching and up to 1 hour, 24 hours and 48 hours after dental bleaching, with a 0-10 visual analogic scale (VAS) and a 0-4 numerical rating scale (NRS). The color was initially evaluated and 1 month after bleaching, with the Vita Classical and Vita Bleachedguide scales, and with the Vita Easyshade spectrophotometer (ΔE and ΔE2000). The absolute risk of TS in both groups was assessed using Fischer's exact test. The intensity of TS was calculated using the Mann-Whitney and ANOVA tests of two factors with repeated measurements, for the NRS and VAS scales, respectively. Comparisons between the times within each group were performed with the Friedman test. The color alteration between the groups [units on the Vita Classical scale (ΔSGU, ΔE and ΔE2000)] was compared with the Student's t test. The significance level was 5%. There was no statistically significant difference between the groups for the absolute risk of TS (p = 1.00) or for the intensity of TS (p > 0.05), in both scales and in the evaluated periods, with the intensity of TS being higher during the bleaching procedure and up to 24 hours after, with a reduction 48 hours after bleaching. The dental bleaching was effective (p < 0.001) in both groups, with no statistical difference between the groups when the color was evaluated through the Vita Classical scale (p = 0.75), Vita Bleachedguide (p = 0.80) and Vita Easyshade spectrophotometer [(ΔE p = 0.64) and (ΔE2000 p = 0.40)]. Co-administration of Ibuprofen and Caffeine prior to dental bleaching and up to 48 hours after, did not reduce the absolute risk or intensity of TS and did not interfere in the efficacy of dental bleaching.
