Abstract
A doença de Chagas, causada pelo protozoário Trypanosoma cruzi, permanece como um relevante problema de saúde pública, especialmente nos países da América Latina. Apesar dos avanços no controle da enfermidade, a transmissão congênita persiste como um desafio, particularmente em áreas não endêmicas, onde a subnotificação e o diagnóstico tardio são frequentes. Este estudo teve como objetivo avaliar a soroprevalência da doença de Chagas em gestantes e puérperas atendidas no Hospital Universitário Materno-Infantil (HUMAI) em Ponta Grossa, Paraná, bem como investigar a presença de T. cruzi em saguis exóticos (Callithrix spp.) capturados, considerados potenciais reservatórios naturais. Amostras de sangue de 362 gestantes e puérperas atendidas no HUMAI foram submetidas à triagem sorológica por ELISA, padronizado com extrato proteico de epimastigotas de T. cruzi. Treze amostras apresentaram reatividade inicial, porém todas foram classificadas como não reagentes após testes confirmatórios realizados pelo LACEN-PR. Paralelamente, foram investigados 53 saguis por meio de esfregaço sanguíneo e PCR. Não foram observadas formas tripomastigotas de Trypanosoma. Entretanto, foram identificadas inclusões eritrocitárias sugestivas de outros hemoparasitas, como Plasmodium sp., por exemplo. A PCR foi positiva para T. cruzi em 5 (9,3%) amostras de DNA extraído de músculo esquelético dos saguis. Os resultados indicam ausência de soropositividade entre as gestantes e puérperas avaliadas e confirmam a presença de T. cruzi em 5 dos 53 saguis capturados. O monitoramento contínuo se mostra necessário, considerando a circulação de triatomíneos no Paraná, a presença de saguis infectados em áreas urbanas e o risco de transmissão vertical em gestantes oriundas de regiões endêmicas. O estudo reforça a importância da vigilância integrada e da adoção de protocolos de triagem pré-natal, mesmo em áreas não endêmicas.
Chagas disease, caused by the protozoan Trypanosoma cruzi, remains a significant public health issue, especially in Latin American countries. Despite advances in disease control, congenital transmission persists as a challenge, particularly in nonendemic areas where underreporting and late diagnosis are frequent. This study aimed to evaluate the seroprevalence of Chagas disease in pregnant and postpartum women attended at the Hospital Universitário Materno-Infantil (HUMAI) in Ponta Grossa, Paraná, Brazil, as well as to investigate the presence of T. cruzi in captured exotic marmosets (Callithrix spp.), considered potential natural reservoirs. Blood samples from 362 pregnant and postpartum women were submitted to serological screening by ELISA, standardized with a protein extract of T. cruzi epimastigotes. Thirteen samples showed initial reactivity, but all were classified as non-reactive after confirmatory tests performed by LACEN-PR. Concurrently, 53 marmosets were investigated using blood smears and PCR. No trypomastigote forms of Trypanosoma were observed. However, erythrocyte inclusions suggestive of other hemoparasites, such as Plasmodium sp., were identified. The PCR assay for T. cruzi showed positive results in 5 (9.3%) DNA samples extracted from the marmosets' skeletal muscle. The results indicate an absence of seropositivity among the evaluated pregnant and postpartum women and confirm the presence of T. cruzi in 5 of the 53 captured marmosets. Continuous monitoring is necessary, considering the circulation of triatomines in Paraná, the presence of infected marmosets in urban areas, and the risk of vertical transmission in pregnant women from endemic regions. The study reinforces the importance of integrated surveillance and the adoption of prenatal screening protocols, even in nonendemic areas.
