dc.description.abstractChronic obstructive pulmonary disease (COPD) is characterized by dyspnea and exercise intolerance. The characteristic of the disease trend to become the patients sedentary, and triggering a deconditioning cycle. In other populations, caffeine (CAF) has showed to be an ergogenic resource, however, there are no studies in COPD. Therefore, the aim of this study is to verify the influence of CAF in exercise tolerance, heart rate variability (HRV) and heart rate recovery (HRR) in patients with COPD. Ten patients with previous COPD diagnosis were invited, in which each patient performed five visits to laboratory. In first visit, the participants answered questionnaires and were measured the anthropometrics, HRV, six-minute walk test and performed one incremental test to familiarized to ciclossimulador. In second and third visits, they performed incremental test with CAF ingestion (5 mg.kg-1 ) or placebo (PLA) (cellulose). In fourth and fifth visits, they performed constant load test with 60% to maximal power in PLA incremental test, but with CAF or PLA ingestion. During and after the tests, heart rate (HR) and peripheral oxygen saturation were measured. Time to exhaustion, HRV and HRR variables in both tests were recorded. Additionally, five patients were invited to perform a pulmonary function test with CAF ingestion, to verify bronchodilator effect of the CAF. An increase, but not significant, was observed in the time to exhaustion of 0.9% in the incremental test and 12.1% in the constant load test (p > 0.05). There was not significant difference between the autonomic variables, with the exception of the ΔHR60s, which was lower in the CAF condition during the constant load test, compared to the PLA condition (PLA = 15.3 ± 8.0bpm vs CAF = 11.4 ± 7.3bpm; p = 0.02). There was no effect of caffeine on forced expiratory volume in 1s (FEV1) (p > 0.05), and caffeine did not change the behavior of peripheral oxygen saturation during the constant load test (p >0.05) and delta of maximal tests did not correlate with the of ΔFEV1 (p >0.05). These findings suggest that caffeine ingestion did not have an ergogenic effect in COPD patients, because a lack of effect in variables of the respiratory system, although is safe in autonomic aspect.pt_BR
dc.description.resumoA doença pulmonar obstrutiva crônica (DPOC) caracteriza-se por ser uma condição onde os pacientes possuem dispneia e intolerância ao esforço. Essas características tendem a tornar os pacientes mais sedentários, formando um ciclo de descondicionamento. Em outras populações, a cafeína (CAF) se mostrou como um recurso ergogênico interessante, todavia, não se tem estudos na DPOC. Portanto, o objetivo do presente estudo foi verificar a influência da CAF na tolerância ao esforço físico, na variabilidade de frequência cardíaca (VFC) e frequência cardíaca de recuperação (FCrec) em pacientes com DPOC. Dez indivíduos com diagnóstico prévio de DPOC foram recrutados, sendo que cada participante realizou cinco visitas ao laboratório. Na primeira visita os participantes responderam a questionários e realizaram avaliações antropométricas, a mensuração da FC para análise da VFC, teste de caminhada de seis minutos e um teste incremental máximo de familiarização no ciclossimulador. Na segunda e terceira visita foram realizados os testes incrementais com ingestão de CAF (5 mg.kg-1 ) ou placebo (PLA) (celulose). Na quarta e quinta visita realizaram os testes de carga constante a 60% da potência máxima atingida no teste incremental máximo PLA, mas com CAF ou PLA. Durante os testes e no período de recuperação, foram monitoradas a frequência cardíaca (FC) e a saturação periférica de oxigênio. Os valores de tempo até exaustão nos testes de esforço, bem como as variáveis autonômicas de VFC e FCrec foram registrados. Adicionalmente, cinco sujeitos foram convidados a realizar um exame de função pulmonar, com CAF, a fim de verificar o efeito broncodilatador dessa substância. Um incremento, porém, não significante, foi observado no tempo até exaustão de 0,9% no teste incremental e 12,1% no teste de carga constante (p > 0,05). Não houve diferença significativa entre as variáveis autonômicas, com exceção do delta de FCrec de 60s, que foi menor na condição CAF durante o teste de carga constante, em comparação a condição PLA (PLA = 15,3 ± 8,0bpm vs CAF = 11,4 ± 7,3bpm; p = 0,02). Não houve efeito da CAF no volume expiratório forçado em 1s (VEF1) (p > 0,05), bem como, a CAF também não alterou o comportamento da saturação periférica de oxigênio durante o teste de carga constante (p > 0,05) e o delta dos testes máximos não se correlacionaram com o ΔVEF1 (p > 0,05). Esses achados indicam que a ingestão de CAF não possui um efeito ergogênico em pacientes com DPOC, devido à ausência de benefício nas variáveis do sistema respiratório, embora seja segura do ponto de vista autonômico.pt_BR
