Cisto dentígero acometendo terceiro molar inferior-relato de caso

Abstract

Introdução: O cisto dentígero é uma lesão que pertence ao grupo dos cistos odontogênicos. Esta lesão envolve a coroa de um dente incluso, sendo os terceiros molares os mais acometidos, e está agregada ao dente em sua junção amelocementária, se desenvolvendo a partir do acúmulo de fluidos na cavidade cística, podendo ser de origem inflamatória ou de desenvolvimento. Ao exame de imagem, este tipo de cisto apresenta-se como uma imagem bem delimitada e radiolúcida envolvendo a coroa de um dente incluso. O tratamento consiste em enucleação cuidadosa do cisto ou marsupialização, a depender do caso. Objetivo: Este trabalho tem por objetivo relatar o caso clínico de um paciente do sexo masculino, 23 anos, que foi encaminhado ao HURCG para remoção do terceiro molar e avaliação diagnóstica de um cisto encontrado no exame de imagem. Relato de caso: Paciente do sexo masculino, 23 anos, relatando que foi encaminhado da origem devido a lesão entorno do terceiro molar inferior direito, descoberta após realizar a radiografia panorâmica. Na tomografia computadorizada, conforme laudo e imagens, observou-se uma imagem hipodensa de contornos bem definidos, unilocular, associada ao dente 48. Localizada em corpo mandibular e início de ramo mandibular direito. Resultado: Foi realizada a enucleação do cisto por curetagem e extração do elemento 48 (terceiro molar inferior direito), sob anestesia geral com intubação orotraqueal, com envio de amostra da lesão para o exame anatomopatológico, o qual confirmou a hipótese diagnóstica de cisto dentígero. Após 90 dias, foi realizada nova tomografia da região, o qual demonstrou ganho ósseo e sem recidivas, demonstrando um pós operatório satisfatório. Conclusão: O cisto dentígero é uma lesão comum de ser encontrada na rotina odontológica, e quando tratada da maneira correta apresenta um bom prognóstico, sem grandes prejuízos ao paciente, desta maneira é de suma importância o conhecimento do cirurgião-dentista sobre essa patologia para poder dar correto diagnóstico e tratamento.

Introduction: The dentigerous cyst is a lesion that belongs to the group of odontogenic cysts. This lesion involves the crown of an impacted tooth, with third molars being the most commonly affected, and is attached to the tooth at the cementoenamel junction. It develops from the accumulation of fluid within the cystic cavity and may be of inflammatory or developmental origin. On imaging examinations, this type of cyst appears as a well-defined, radiolucent image surrounding the crown of an impacted tooth. Treatment consists of careful enucleation of the cyst or marsupialization, depending on the case. Objective: This study aims to report the clinical case of a 23-year-old male patient who was referred to HURCG for removal of a third molar and diagnostic evaluation of a cyst identified on imaging examination. Case report: A 23-year-old male patient reported that he was referred from his place of origin due to a lesion around the right mandibular third molar, discovered after a panoramic radiograph was performed. On computed tomography, according to the report and images, a hypodense, well-defined, unilocular image associated with tooth 48 was observed, located in the mandibular body and the beginning of the right mandibular ramus. Results: Enucleation of the cyst by curettage and extraction of tooth 48 (right mandibular third molar) were performed under general anesthesia with orotracheal intubation. A sample of the lesion was sent for histopathological examination, which confirmed the diagnostic hypothesis of a dentigerous cyst. After 90 days, a new tomography of the region was performed, demonstrating bone gain and no recurrence, indicating a satisfactory postoperative outcome. Conclusion: The dentigerous cyst is a lesion commonly found in routine dental practice, and when properly treated, it presents a good prognosis without major harm to the patient. Therefore, knowledge of this pathology by dental surgeons is of utmost importance to ensure correct diagnosis and treatment.
