Tratamento conservador de ameloblastoma: relato de caso clínico

| dc.description.abstract | Introdução: O ameloblastoma é um tumor odontogênico benigno, com tendencia de crescimento agressivo e risco de transformação maligna. Ocorre principalmente na quarta década de vida, em pacientes do sexo masculino, com grande predileção pela região posterior da mandíbula. Comumente assintomático e caracterizado por expansão lenta, em contrapartida, localmente invasivo e infiltrativo. Radiograficamente, apresenta-se uma lesão bem definida, uni ou multilocular, com aspecto de "bolhas de sabão". As modalidades de tratamento incluem excisão cirúrgica, enucleação, crioterapia, radioterapia e quimioterapia. Objetivo: Relatar o caso clínico de um tratamento conservador de ameloblastoma mandibular em um paciente de 17 anos. Relato de caso: Paciente do sexo masculino, 17 anos, exibindo aumento de volume extraoral em terço inferior de hemiface do lado esquerdo e intra oral em região de ramo e corpo mandibular esquerdo. Os exames imaginológicos revelaram área radiolúcida, bordas irregulares, estendendo-se do corpo mandibular, acometendo angulo, ramo e coronoide da mandíbula, associado a dente incluso e reabsorcao dentária. Resultado: Foi realizada a marsupializacao da lesão sob anestesia geral, com envio de material para análise histopatológica e resutlado de ameloblastoma. Após 11 meses, foi realizada enucleacão do remanescente da lesão e acompanhamento da mesma. Conclusão: O Ameloblastoma é um tumor que apesar de benigno pode ser agressivo aos tecidos adjacentes. O cirurgião-dentista deve conhecer essa patologia para saber indicar o diagnóstico e tratamento mais adequado para o paciente. | |
| dc.description.sponsorship | Introduction: Ameloblastoma is a benign odontogenic tumor characterized by a locally aggressive growth pattern and a potential risk of malignant transformation. It most frequently affects individuals in the fourth decade of life, with a higher prevalence in male patients, and shows a marked predilection for the posterior region of the mandible. Clinically, it is often asymptomatic and presents slow expansion; nevertheless, it demonstrates invasive and infiltrative behavior at the local level. Radiographically, ameloblastoma typically appears as a well-defined unilocular or multilocular radiolucent lesion, classically described as having a “soap-bubble” appearance. Therapeutic approaches include surgical excision, enucleation, cryotherapy, radiotherapy, and chemotherapy. Objective: To report a clinical case of conservative management of mandibular ameloblastoma in a 17-year-old patient. Case Report: A 17-year-old male patient presented with extraoral swelling involving the lower third of the left hemiface and intraoral enlargement in the region of the left mandibular body and ramus. Imaging studies revealed a radiolucent lesion with irregular margins extending from the mandibular body to the angle, ramus, and coronoid process, associated with an impacted tooth and evidence of dental root resorption. Outcome: Marsupialization of the lesion was performed under general anesthesia, and the surgical specimen was submitted for histopathological examination, which confirmed the diagnosis of ameloblastoma. After an 11-month follow-up period, enucleation of the residual lesion was carried out, followed by continued clinical and radiographic monitoring. Conclusion: Although ameloblastoma is classified as a benign tumor, it may exhibit aggressive behavior toward adjacent tissues. Therefore, dental surgeons must be knowledgeable about this pathology to ensure accurate diagnosis and to select the most appropriate therapeutic approach for each patient. | |
